Fill this in before you travel or step away, and hand it to whoever covers for you. The rule behind the whole page: if it is only in your head, it has not been handed over. Fill in the boxes even when they seem obvious — what is obvious to you is exactly what gets forgotten by someone else.

1. Medications
The current medication list is printed, up to date, and attached to this sheet.
Where the medicines are kept at home is written as a description, not a gesture ("the top drawer in the hallway").
Anything recently stopped is written down by name, so a stopped medicine cannot be given.
Names are exactly as printed on the boxes, not as the family pronounces them.
Any short course ending during the handover has its end date and the person responsible for stopping it.
2. This week's appointments
The appointment, the day, the time, the place, and who is going.

4. Daily routine
Usual waking and sleeping times, and when meals happen.
What needs help (moving, bathing, eating) and what they manage alone.
Keys: who holds a copy, and where it is.
Anything that upsets them when it changes (a particular visitor, a chair, a device).
5. What the clinician asked to be watched — in their words
Copy here, verbatim, whatever the doctor or pharmacist asked you to watch for or report. Do not add a medication observation of your own. If nothing was asked, write "nothing" — a blank box reads as an oversight.

Notes on using it
Agree the handover time before the travel, not on the day someone leaves.
The sheet is updated during your absence: whoever is covering writes down what changed, otherwise you return to a handover that no longer matches reality.
One handover sheet is the reference — not three near-identical copies that disagree within a week.
This file is an organising tool, not medical advice, and it does not replace a doctor or pharmacist. This file has not been pharmacist-reviewed yet.
