One sheet per person. Print it, fill it in by hand, and put it where you look every day. Print a fresh copy whenever a medication or a dose changes and destroy the old one — two sheets on one fridge means a mistake is a matter of time.

Six columns are enough for one person. Write names exactly as printed on the boxes.

Notes on filling it in
Write the dose out in full: "one tablet", "half a tablet", "5 ml" — never "a tablet" on its own.
Anchor times to a habit that already exists (after breakfast, at wind-down) unless the exact clock time is the requirement.
If more than one person gives the medication, put the responsible person's name against each time.
The note column is for logistics only: the box number, or who fetches the next refill. It is not the place to record a new symptom — that goes to the doctor or pharmacist.
This file is an organising tool, not medical advice. Rifd is an organising tool for daily care and offers no diagnosis and no medication decision; any question about a dose, a new medicine or a new symptom belongs to a doctor or pharmacist.
