The thing an appointment loses most easily is the decision that gets spoken out loud and written nowhere. This page has three parts: what to carry before you go, what to write down before you walk in, and what to fill in the same day you leave. Print it and keep it in the appointments folder.
Patient: Date: Hospital / clinic: Going with:
Before — what to carry in your hand
The current medication list, printed, with names exactly as printed and their doses and frequencies.
Anything recently stopped, written down by name — it appears on no other list.
Anything taken without a prescription, plus supplements, on the same list.
The boxes themselves where a dose is unclear.
The last report or result, and any imaging that was requested.
The file number and insurance details if needed, and a pen.
Before going in — three questions to write down
Write them first. The question you did not write down gets forgotten in the room.
Question
Answer, as it was said
Has anything changed in the medications, and exactly what — which drug, what dose, what time?
If we miss a dose, what should we do, and who do we call?
When is the next review, and what should we watch for and report before it?
After — the same day, not by the end of the week
What changed, who updated the record, and who tells the rest of the family.

Next appointment and its date
Who is going with them
Old sheet destroyed?
Reminders updated in one place?
What this checklist does not do
It decides no dose, recommends stopping nothing, and postpones no review. Every medical decision belongs to a doctor or pharmacist.
It is not a substitute for asking: the three questions are written because they are questions, not because their answers are known in advance.
It does not survive in memory. A sheet you fill in and then leave in a wallet beats a recollection nobody can check.
This file is an organising tool, not medical advice. This file has not been pharmacist-reviewed yet.
